Default I'm not sure GURPS' physics engine can handle this

SM -10

Affliction (SM -10, Always On, Emanation, 10yd range)

I have no clue how this would work. I think the game would crash. I want it to work...but I don't think it has a chance.

Size Modifer is a feature. There are no rules for Afflicting features. You can, however, modify Affliction with Advantage (Shrinking) or (Alternate Form). Neither of which especially break the system.

It would be trivial to include -10 SM in a 1-pt or -1-pt metatrait.

How do you make everything more than 10 yds away grow as well?

This is just the same case as adding Switchable to 0-point feature. That isn't valid.

Afflictions with meta-traits should add the cost of each underlying trait separately. And afflictions with features should use appropriate feature-changing advantage (Elastic Skin for Appearance, Growth or Shrinking for SM and Alternate Form for anything more weird).

Powers p.143 does not agree: it uses Affliction (Alternate Form) to turn people into frogs, and the frog's racial template would be a metatrait. A Liliputian template would work just as well.

An idea that doesn't work if you want indiscriminate shrinking

Don't mix up meta-trait and racial template. Meta-trait isn't directly appropriate as enhancement for Affliction and must be used either as racial template or divided onto elementary traits.

Compare turning every person into humanoid lulipute or frog or just shrinking them without morphology change.

As I said, Affliction (Alternate Form) works fine.

The problem is that if you Afflicted someone with the following metatrait:

Feature: SM-10
Quirk: Catch Phrase: What the hell happened?

You turn humans into tiny humans, and elves into tiny humans, and robots into tiny humans, and buildings into tiny humans.
