Default Grappling with Reach?

Looked through Campaigns, Martial Arts, and Google, but couldn't find anything about this, was wondering if anyone knows about or has encountered this before.

Basically, how does natural reach due to either SM or Stretching affect grappling?

Grappling says you must be in CC with your opponent, but it seems to me you could grapple them at whatever distance you can reach them. A giant wouldn't have to be standing above you to grab.

I would let them use their full reach if they used an All-Out Attack

For a grab certainly. Some combat options, however, would still require actual body contact - bear hugs being the most obvious one.
