Looks like the Man With No Name, the Clint Eastwood character from A Fistfull of Dollars/etc.

Wouldn't he need ambidexterity, or at least Off-Hand Weapon Training(Brawling)?

That would work, but bear in mind that these stats are not meant to be comprehensive. They're not invalidated just because they exclude a detail. In fact, many writers like to exclude some real dead give-aways.

Only if you're using the highly optional off-hand for unarmed combat rules.
