I've done this using Microsoft Word and Excel and mail merge...

I made two which are fillable PDFs. One of the Wizard Character pad from the Legacy Edition and one custom one for when the PCs need a bunch more space to make notes and stuff.

I pretty much use post-it notes or 3x5 index cards, but I'll follow the thread with interest!
