So I've recently discovered that I've been playing the game wrong for years (I guess whoever taught me didn't read the rules) so I've been trying to clarify a few things. A thing I've learned is that when you take a card you are carrying to replace a card in play, those cards just switch (I've always discarded the in play one). My question is, say I have a +1 helmet in play but I want to replace it with a +2 helmet from my hand. Does the +1 helmet go to my carried items? Or is it discarded?

You never discard Items unless a card tells you to or you are selling them. (Or you're playing a one-shot.) In your case, you would simply unequip the +1 Item by turning it sideways and equip the +2 in its place.
