Why did you take Reliable +6? Do you want to have an effective skill of 16?

When teleporting to a known location you ignore Preparation bonuses/penalties. When teleporting to a known location you, if you have Eidetic Memory you roll at +5 or +10 if you have Photographic Memory. Also consider Cosmic: No Roll Required +100%.
