A few claifications on Dungeon of Gender Bending?

I have a few questions on how people are reading the rules for the Dungeon of Gender Bending.

1) Does every sex change even when you GUAL cause a penalty the next combat or is it only on entering the dungeon?

2) If no Wishing Ring is used do all items that are sex based get discarded unless they have a gold value and can go in the belt? Such as a Hireling that can only be with Females.

3) When you exit the dungeon do you just get the sex change from what you currently are or is it back to what you were when you entered?

4) Does the penalty apply when exiting as well?
