There were intercalary months in the Egyptian, Babylonian, Hebrew, and Roman (Julian) calendars -- which was the medieval European calendar until Pope Gregory reformed it in 1582. All those fantasy RPGs in pre-modern settings might well throw one in.

I've never seen anything about that happening in the Julian. My understanding is that it had a leap year day every four years, period, which caused the calendar to lag by 3 days per 4 centuries; the only "intercalation" that I know of is the eleven days that were stuck in during the Gregorian upgrade (and rioted over!). Do you have details on the intercalation you refer to? I'd be interested in seeing them.
