What examples do you have in mind? Koshai the Deathless deliberately hid his heart in an egg, in a duck, on an island. So he definitely knows were it is. D&D liches usually know where they put their phylacteries. What are you thinking of instead?

Having a whole set of plot coupons would reduce the limitation on Unkillable to something like 5 or 10 percent and raise the mitigation on Terminally Ill to 80%

I remembered Voldemort exactly because not only he has more than one phylactery (six to eight, depending on the time of the Harry Potter series), but also the destructions of said phylacteries don't kill him instantly: that only makes him mortal.
