Books with campaign planning forms.

I'm trying to make a list of all Third and Fourth edition GURPS books that have campaign planning forms. Some campaign planning forms exist in some books where you might not expect to see them, like GURPS Cops, for example. The campaign planning forms ask questions which are specific to that genre, so they are all different. For example, the one in GURPS Cops asks if cops or criminals have psionics, as well as other mind expanding questions to consider when designing your game. So what books can you name which have campaign design forms in them?
