My preference for having PC able to do extreme, borderline impossible things like that is to give them enough points to be able to have the skill levels to do them, rather than change the rules to make them to easier to do. It just feels more awesome to me to have the PCs be exceptional by design, rather than the universe being tilted toward allowing things like that.

Either do I...

But being able to perform first aid on oneself while driving at full speed in a car chase and reading the map at the same time would logically give such a penalty that it would require an incredible base skill. I'm not sure that Jason Bourne would have so many character points in a secondary skill like First Aid*...

So, for heroic genre, the simplest way to handle things is often to ignore penalties that are supposed to make things realistic (when not very harshly realistic).

_____

*Secondary for Jason Bourne. He is first of all a fighter, not a medic.

Cracked has some examples of insane self-surgery that worked here: http://www.cracked.com/article_19218...ly-worked.html

I lean this way as well. For 'cinematic' type games, let the skills go through the roof (and the TDMs too!).

I've done simple first aid on myself, no doubt everyone has. Granted for those things (remove splinters, 2nd degree burns, etc) I wouldn't require a skill roll.

I've also: stitched a wound on my left arm (4 inch long 1 inch deep gash); removed spines from under my knee cap (13 spines ranging from 1-3 inches); put dislocated fingers and shoulder back into place; treated minor eye injuries; treated 1st, 2nd, and 3rd degree burns on myself (heat and chemical burns over the years, all smaller than 1/4 inch radius**); treated sprains; bound a fractured toe (several times); identified and treated a black widow bite; identified and treated non-poisonous snake bites; and... I think those are the only ones I'd consider having needed a skill roll.
I would point out that all of those were probably <1 HP damage, and thus would suffer no penalty under Kromm's rule. So it still seems appropriate.

Also, what the heck are you doing to yourself?!?

The gash certainly wasn't and no 3rd degree burn should be.

Surviving.

The spines were an accident as a kid (10 years old), most of the rest were work related (construction and industrial machining). I hate going to hospitals.

My mom's husband is like that. Now he walks with a limp and moves like a snail from all the injury induced arthritis from yesteryear and inadequate doctor listening.

I'm gonna end up that way... but it'll be due to all the getting hit by cars.

My left hip and knee are already all kinds of arthritic and I'm only 40.
