In DF you can treat shock at -10 for instant result.

But in a realistic campaign I would limit bandaging to the normal -9. (Though if you use my extended time taken penalties I allow any amount.)

For treating shock I would require the appropriate TL tools to get the lower base time.

As for time reduction in a realistic campaign, I would allow it as for total work time used by the medic, but the target gets the benefits only after the full time. Thus at -5 you could treat 2 patients at the same time and so on.
