For me, that depends on the campaign.
For the shared campaigns I've GMed involving Yrth (Strategicon conventions, home games, online games), I've pretty much gone by GURPS Banestorm (or other including earlier supplements dealing with the same world). For a hard science fiction galaxy I've run (Strategicon, home games) and published stories about, nobody but me knows where humans originally came from. In a shared TL4 fantasy campaign centered in a large city (online games), the origin of different races/species varies greatly, from "they originated a long time ago and far, far away" to "they were created by magic." In a couple of supers campaigns (Strategicon conventions, home games, online games), most originated on Earth--but some came from a different world/dimension. Etc.
