It's the GM's option whether transformations cost points or not. If it "doesn't make sense" to you, that's your problem, not the rules.

Traits acquired in play Just Happen, and change your point total. You do not "owe" points for good things or get points back for bad ones . . . you just change in value. If what you get happens to be disadvantageous, you can opt to pay points to buy it off. Or you can hope it'll go away, get cured, etc. for free – that's your gamble to take.

The mutable point totals option boils down to "found is found". Their point total will go up as you earn experience or train but if they get their legs blown off in a tragic golfing mishap, so be it.

This implies it's possible to buy off physical disadvantages, such as losing that leg. I'm assuming that's not true.

Yes. Spending the points just guarantees your access to whatever leg replacement abilities are available in the game.

Exactly. "Cyber-leg? No worries . . . $300,000 please." "I don't have $300,000." Then you spend 20 points to buy off Missing Leg and the GM points out that at TL9, 20 points of Signature Gear is worth $300,000. "Fortunately, there's a slot open for an experimental model. You sign the waivers and it's yours."
