What examples do you have in mind? Koshai the Deathless deliberately hid his heart in an egg, in a duck, on an island. So he definitely knows were it is. D&D liches usually know where they put their phylacteries. What are you thinking of instead?

I remembered Voldemort exactly because not only he has more than one phylactery (six to eight, depending on the time of the Harry Potter series), but also the destructions of said phylacteries don't kill him instantly: that only makes him mortal.
