That would probably only be a problem if you were going for the suffocation option. If a character goes that route, she'd be best off wearing some denim or leather pants/shorts - but, of course, virtually every character who does the crotch-grapple is either wearing a skirt or leotard (or both). On the other hand, if she's squeezing tight enough that the enemy can't breathe, their mouth probably isn't going to be able to move. In that case, so long as she's wearing something (teeth are a bit sharp, after all), she should be alright.

If the character is male and doing the crotch grapple, however, there's a decent chance he won't be for long. But I'd prefer not to think about that.
