In the case of Giant Skunk, cards in your hand don't count, they don't count as anything but cards until they are in play.

As for Medusa, you have to look at how Death works. When you die, you put your cards on the table with your equipment and then the other players take one card each. Medusa says, in an indirect way, that you skip the 'loot the body' step of death. So you just discard everything, cards in hand included.
