All in all, it's a fantastic video and a good primer on FMA, so do yourself a favor and watch the whole thing.
Some qualify, many are facilitated by a weapon-enhanced grabbing parry, or follow on to a more intricate attack with the weapon using Armed Grapple or Judo with the weapon to enhance control points.

Some of the could be that Guro Inosanto is skilled enough to have Extra Attack, or have bought off Rapid Strike combos or gotten combinations that are defend-attack-attack, with some of those being grapples and ending with a disarm.

The instant disarm section in Technical Grappling tries to have my cake and eat it too. I talk about the already-established mechanic for this, which is to attack the weapon (at a penalty, with an additional -2) and then make an immediate Quick Contest, and victory disarms him. You're mostly talking -6 to -8 to hit, and then a QC . . . low percentage.

The second method is to grapple, and then win a QC of Arm or Hand lock or Disarming vs Retain Weapon.

Grumble grumble, it's unclear in the text, but the grapple and then the QC are both attacks, so technically should require an All-Out Attack or Rapid Strike or with Extra Attack to do them all in one turn. Or, use the aforementioned Grabbing Judo Parry, followed by the Quick Contest of Disarming/Arm or Wrist Lock vs Retain Weapon.
