Nope, I made these to use with good ole' pen and paper. If you have access to a licensed version of Adobe Acrobat, you should be able to create forms.

The software I used to make these is quite old and the automation possibilites to make form fillable pdfs are quite limited, thus I will not be adding this feature for the forseeable future, I am sorry.
