Foggoth (Smoggoth in Italian?) ability in multiple monsters fight

I m playing munchkin cthulu where "goth" monsters can be added without the wandering monster card. My question is: Since smoggoth says "items dont count against him" how do i manage the fight with him and another goth? Do items dont count for both or what?
I have the italian version of the game maybe it has a different name i dont know.
