I just have a question about bad stuff. When is it activated? If you lose the battle or happens as soon as you start?

When you lose a battle and fail your roll to Run Away from that monster. See "Running Away" in the rules.

Bad Stuff is what you suffer when you fail to Run Away from a Monster after losing combat.
