Is up a possibility? Acrobatic Dodge to vault the rhino, assuming sufficient Jump to clear.

Yeah, I guess that works, if getting Trampled by the thing is better than getting Slammed by it for some reason... Trampling usually does more damage, while Slam does a little damage and knocks you Prone if successful, so diving Prone (ie: giving it what it wanted by Slamming), possibly into a Trample, to avoid a Slam seems like it'd usually be a little counterproductive IMHO.

If the very cinematic Martial Arts rules that allow Jumping Dodges and Evades are already in play, and you've got enough of a maximum high jump using the formula for no prep at all to clear the highest point on the thing, then sure.

The primary problem with a rhino slam is the pointy impaling bit on the front end.

I'd say the problem with rhino slam is the 2 tonnes of rhino running at 40+ km/h behind the impaling bit on the front end that is the problem with the slam.

If it hits you with the slam, is there any chance you're not going to wind up prone?

You could Wait, take an Attack maneuver then use Extra Effort in Combat to take a Great Step, move two hexes, and then attack the Rhino as it charges by. If you aren’t using Extra Effort in Combat rules, then your best play is to not be in the path of a giant charging rhino in the first place. Fortunately, they’re hard to miss, so you can start moving for cover right away.

It could happen, sure. Slams are somewhat unpredictable, and I don't know the target's HP, DX, etc.

On the other hand, now that I actually read the Trample rules (4e Campaigns p 404), it turns out that the target doesn't even have to be Prone if the Trampler's SM exceeds his by 2+, so unless the target is also quite large it can just target him with the Trample in the first place anyway if it wants to, and a Dodge-N-Drop between its legs is probably the best response to that (if you can't chambara-jump over it), at least if it seems to have enough momentum to carry its whole body past the hex you'll wind up Prone in (because otherwise, yikes next round...)

Presumably you'll get a turn before it does, so you have a chance to get out from under, don't you?
