The staff of mitra grants +4 against undead. Does this +4 apply to EACH undead monster in the combat or does it apply just once even if undead are added to the other undead monster initially in combat?

Related to the undead, the apples of Deketa is a oneshot item card that grants +3 but isn't effective against undead enemies. However, can this card be used to aid an undead monster fighting the munchkin since the munchkin isn't undead and get the +3?

Sure. It just can't be used against Undead.
