About that. Though I think it depends a bit on how "accidents" are treated. These days I think the trigger point is if your society won't treat you as a criminal for it, you kill them, otherwise you don't have to. If accidents don't draw anything worse than a negative reputation for being careless, you should try to arrange to have one, but if you fail and knock your opponent unconcious, you don't have to finish him off and face a murder charge for it. Similarly if your society is OK with using lethal force to defend yourself in a tavern brawl, you should be trying to kill your foes there too.
