As it is, unless you add Malediction, a beneficial Affliction may end up resisted by a target even if they want it. But +100% seems steep if you want nothing else from Malediction's mechanics.
Would Cosmic: Rules exception (50%) be fair to allow targets to waive resistance if they wanted to do so? Any downsides?

Had t think about it but I agree.
Unless your casting it on really unhealthy people your likely to succeed anyhow. I dont see Cosmic as unbalancing or a big deal.

Eh. I'd allow any character without a specific restriction (e.g. Magic Resistance) who's aware of the attempt to waive resistance at will, no enhancement required.

IIRC, a willing and aware subject can already decline to resist. I may be leaving out an enhancment that lets "the attacker" make the roll by +0

It is. The level is one factor in calculating the modifier; it is not the entire modifier.

Only if you assume the roll must be a HT roll, despite the text that says it's a "HT + 1" roll. The fixed and immutable portion could well be considered the "base", while the variable part is the modifier. <shrug>
