For something like creating a sword the duration would determine how long it was around. For creating energy or matter to cause harm (e.g. adding the Damage modifier) this would last long enough to cause damage and then presumably disappear. Destroying something usually uses the Damage modifier as well but I could see you useing it to destroy chemical bonds in a subject (H20 to Hydrogen x 2 and Oxygen x2) in this sort of regard its a poor man's alchemy. Do note you'd need Transformation to turn one thing into another though!

No. You cannot create anything
permanently! You could add a huge Duration though to make something
effectively permanent. So adding a Duration of 100 years (+120 energy) would be permanent in most campaign settings.

You can use a Lesser Destroy Mind effect to remove a memory. If it's a single memory I wouldn't require a Duration - anything really complex would require a Greater effect possibly with Altered Traits (Delusion - False Memories) plus Duration. I've treated memories in my campaigns sort of like damage, eventually you might be able to 'heal' them. I'd add Duration to ensure they couldn't come back. As a GM YMMV.
