These are pretty cool! At least as a thought exercise, and for anyone who honestly wants that much realism in a game. :)

Should kidneys also get a penalty to bleeding rolls? Maybe they aren't as bad as the liver or lungs, but they are filled with blood vessels. Maybe -1?

And what about the spleen? That isn't "vitals" but it is pretty vascular, right?

Now, if you want to get absurdly specific, there is the pancreas, which if damaged badly enough could potentially give somebody diabetes. ;) But that's probably taking the details too far, and the thing's pretty small.

Yes, probably. -1 sounds good.

What would the effects be? Just more bleeding? Can you survive a badly damaged spleen? I really don't know enough to even make an educated guess.

This reminds me of another issue with these vitals: for some of them you should have to make a Physiology or IQ-Based Combat Skill roll in order to target them intentionally, right?

You also get into diminishing returns: if I include the pancreas, but it's both harder to hit and less damaging than the kidneys, who would ever target it?

There definitely is such a thing as too much detail, but the point of this thought experiment IS to take things too far. :)

From the most basic of sources:
Ruptured Spleen
Various abdomenal organ traumas

Sounds like injury to the spleen (or kidney or pancreas) is worrying but not devastatingly so unless there's a ruptured artery involved. I'd probably call that the basic Vitals effect, but YMMV.

Thanks! Those are great sources. The second one especially seems to varify a lot of my assumptions.

As long as you're having fun going too far...

Targetting obscure organs could involve a skill roll, yeah. Good idea.

The spleen is a bleeding target. It bleeds a lot (like a kidney, I'm guessing) but other than that losing the entire organ usually won't kill you. Apparently some children and occasionally older adults who have it removed end up with much weaker immune systems, though. It is located in between the stomach, diaphram, and ribcage, so it can be hit by a blow to the left side, or if the stomach is hit with piercing, cutting, or crushing then on a roll of 1 on one die, the spleen is also hit at 1/3 of the damage done to the stomach? The organ is the size of a fist.

The pancreas is maybe a bit smaller than a hand. It wouldn't bleed faster than "torso" but losing it entirely would cause diabetes and problems digesting food in general. To destroy it with a physical attack would also require damaging the intestines and or stomach, so they'll have digestive problems anyway!

Now about the gall-bladder, cecum, and appendix... ;)
