Heading out to the Oort to avoid orbital congestion is a little extreme. There might only be enough room for approx. 3 ships to leave geosynch for hyperspace at one time but even busy airports rarely have more than 3 airplanes taking off at exactly the same time.

Beyond Earth orbit, 20K km is very much a flyspeck.

In short, I don't really think you have a problem.

Off the plane of the star system, there's really plenty of space to play with.

If interstellar travel is an organized activity, standard traffic rules could be applied to specify where you're supposed to jump to based on the relative characteristics of any point of origin.

Something that I decided was that entry/exit into hyperspace could not happen if there would be a collision. Use whatever reason you want, but mine was that the entry/exit point can not form if there is a physical object other than the ship creating it inside the sphere (in realspace or hyperspace).

Yeah, I'm thinking that when I get to run a space campaign with FTL, I'll declare that the entry sphere must be relatively empty both in TS and HS, and if it is not quite empty, the ship will have to expend some energy on 'emptifying' it (which is affordable in the interstellar/hyperspace medium, but so prohibitive for anything with a density of even an atmosphere that it's just too inefficient to weaponize). This has the side effect that a ship in HS can figure out if its exit sphere in Truespace is empty or not.

I guess it's not a big deal if the de facto requirement for forming an exit sphere is something like being 50au from the Sun (alternately, an exit sphere could be formed 'nonconsensually' if a ship in HS tries to approach another high-density in object in TS).
