I would give the centaur all human hit locations above the waist (with human-sized armour) plus a horse Torso, Forelegs, Hindlegs, Tail, and Groin (with horse-sized armour). Also see the Quadruped hit location table p. B553.

Isn't 14 or 15 hands high a typical warhorse height? I'm not sure if I recognize the other differences, like a thicker barrel and shorter, studier legs, in fantasy art. Anyways, GURPS has a house policy of not differentiating armour costs and weights between things with the same SM and racial template.
