The question of whether the specific recipient can refuse the item is not addressed, but my reading of the rules says that he cannot -- he's getting the Big item, for good or ill. This is something we should clear up and get into the FAQ.
So if I hold a Big item in my hand, and have six cards in hand, I cannot give that Big item to the lowest level player as Charity if they have a Big item in play? That seems odd, since I can give Race or Class cards as Charity, can't I?

Or have we been playing the Charity rules entirely wrong?
