I think the 14- for a PHD is reasonable but would not balk at 12- for a doctor, especially one who has been practing awhile and may have let some skills lapse.

And yet the ER docs are NOT the top job in the field. Sure, they're better than the GP, but it isn't the high demand only for the best field requiring a high skill score from would indicate. At least on physician. And they don't occupy that place in the medical pecking order either.
I wouldn't give the ER guys extra high scores on anything but first aid. Most of the time they're treating the obvious in a stressful and intense situation.
