What you wrote up sounds good to me.
To answer your questions: I have not done this myself, so I am glad you did. And you are correct: being in the hex with a lit torch is not a problem, unless you something flammable spilled on you.
The only change I would suggest is #7. Typically I think of the 3 MA for moving carefully through a hex with something large like a body. I would just let folks run/walk through a hex at normal cost that had an lantern.
