The best illusions do not just appear out of thin air. The clever wizard will create them out of sight or around the corner when possible so that that their approach will seem natural.

Note how illusion doubles work.

transition from smoke, but with no extra smoke to dissipate.

They just appear suddenly, but there's also a slight subtle sound effect due to the air movement.

I'm going from the 'behind a curtain' one, because I feel they appear with some sort of undefinable shift and the feeling that they had been there all along.

They "resolve" into being, sort of like how a low-poly model in a videogame is eventually replaced by the high-poly versions. The illusion starts out general and then takes on specific form and features as it comes together in the caster's mind. Experienced illusionists make this transition happen very quickly, and often have vividly imagined favorites that they can call into being fully formed.

For larger summons,
There is the loud sound as of fabric tearing as reality is momentarily ripped apart and the summoned being enters the world. |

In my story, a combined artifact/magic device in the form of a trapped chest, summons three myrmidons.
The summoning itself does not have any extra effects, but the chest was also trapped to release smoke, giving the summoned pirates a chance to get the drop on their opponents. |

Everyone knows Summoned creates appear to the tune of the GunsN'Roses intro to "Welcome to the Jungle".

I must agree with those who make this the magic user's decision. If spectacular crisis intervention is required then make with the light show, smoke and sound effects. If sneakiness is more important, let the dim beast seep slowly out of a well of shadow. Of course, if it is the DM's illusion, anything goes!

Re: How Do Summoned Beings and Illusions Appear in Your Game?
No arguments there; it is up to whoever is controlling the character casting the spell. The question that is the subject of the thread is meant to encourage players to add some colour to their playing instead of, "I cast an illusion of a Myrmidon here."
