Yes. It's as a result of combat.

The text of Captain Quirk does not particulaly say "kills", but "removes from the galaxy". Does it mean only killing or any kind of removal (e.g. Pollymorph potion)?

It means "kills." No one gets levels for merely banishing him to an alternate dimension.
