Random question

A monster's bad stuff says I should get rid of all the cards in my hand or all of my small items. At the time I have no small items in play. Can I choose the second option and throw away nothing?

No. Bad Stuff always has to be bad. If there are two options, where Option A applies, but Option B does not, you have to take Option A. If neither option applies, you got lucky.
