I love 4E with all my heart, but whenever you start modding disads, the mechanics emit odd, creaking noises.

Mitigator explicitly requires ongoing therapy and/or an easy-to-break device. The idea is that therapy makes your day-to-day life notably trickier or more costly, while a device is more fragile than the human body. If this isn't true of your solution to your disadvantage – if that bionic eye doesn't require a special coolant or a regular tune-up, or perhaps extra-careful treatment to avoid malfunction – then it isn't a Mitigator.

As for disadvantage + not disadvantage constructions, please note that the post in which I was quoted is going on six years old. At that time, most of what's now out for GURPS Fourth Edition was still a work in progress. At this stage, enough art has been published to revise that opinion to reflect the state of the art: Such constructions are fine when they don't give a bigger discount than an equivalent Mitigator and meet with the GM's approval.

Where? I seem to recall that there's actually a template with Mute (Only While Insubstantial), as opposed to Mute plus Not Mute.
