In hindsight, Balanced Attack might be a better name. You then avoid "Attack != attack" confusion and get a clear spectrum:

All-Out Attack
Committed Attack
Balanced Attack
Defensive Attack
All-Out Defense

On the other hand, it might add a potential for confusion when combined with unbalanced weapons.

Poured over so many books trying to maximized damage... this post reminds me of similar. Best way to maximize damage I've seen for melee characters in cinematic fantasy and still be "Human" for the most part is Power Blow.
