As others have said, low-tech fire is unlikely to inflict an immediately fatal wound in a few seconds like a club or a stone arrow can. On the other hand, it is terrifying, and inflicts wounds which are hard to heal even with 20th-century medicine. Don't forget the rules for Setting Things on Fire! And Fright Checks may be in order, and prevent the burning character from doing something sensible like stopping, dropping, and rolling.

From high school, I recall that lung-and eye-related problems were very common amongst the longhouse cultures of the Northeast. Spend a Great Lakes winter in a bark-and-poles house heated by open fires, with plenty of furs but not much of a local fabric tradition, and you will probably learn to tolerate the smoke even if your eyes burn all the time ...

Actually, we don't really. Ordinary levels of smoke are mostly long term toxic, and thus kill you after you've already procreated.

They affect pregnancy, infants, infant mortality and childhood development, all of which is selective.

Is that a thing? Where does that come from?

Do you mean from an Affliction of Extra Life (as in the Raise Dead learned prayer)?

GM tricks for digging players out of holes 1.0 - trading a death for some kind of permanent consequence.
