So the "Out to Lunch" card can't be applied if one monster was automatically killed or a card "Pollymorphed" was applied? But it is possible if there were several monsters and all of them were automatically killed or removed, and so that the player receives only two cards?

Yes, that ruling has been superseded.

If the last monster is removed from a combat, cards that say "play during combat" may not be played, because combat is over.

What actions mean to remove? Wizard's pacified? Opened the monster everyone saw, I folded the battle ended?
In other cases (when there is another monster) removal of monsters - reversible or not? That is pacified one monster, and then "went to lunch" or battle were taken - what will happen?

If something causes a monster not to fight anymore, that monster has been removed from combat.

If there are no other monsters still fighting, then the combat is over.

If other monsters are still fighting, then "during combat" effects may still be played, and may or may not have an effect on the removed monster depending on how it was removed and what the rules said. I can't give you an overall "this is what happens" rule in that case because there are different ways of removing monsters from combat.

I am rereading the FAQ and found this:

Q. Does the "reasonable time rule" apply to defeating a monster without killing it, or just to killing it?
A. Any sort of defeat. If you defeat it without killing it, other players still have a reasonable time to play an applicable card to frustrate you. However, if you removed the only monster from the fight (with Magic Lamp, for instance), they cannot play Monster Enhancers or other similar cards that affect a specific monster, because there isn't one there to fight, and they can't use any special rules for bringing in monsters (such as the Shark rules from Munchkin Booty) that depend on having a monster in the fight. They can play Wandering Monster to bring in an entirely new monster, but they must do it at once.

Does the new rule affect this? I assume the combat ends immediately and this is just an oversite.

If the person draws a monster who is insta-killed by flame/fire attack, and the player is level 9 with flame/fire attack. Is this an instant win, or do people have time to react (wandering monster/friendship potion/etc)?

It’s an instant win. Good job on exploiting a combo. If you’re worried about a player at Level 9, don’t let them get to Level 9.
