Well he has to, otherwise it isn't a horribly slow murder, it's just eternal torment. For it to be murder, he has to kill you. We don't see the death, but presumably poor Jack will eventually die. He won't let you commit suicide, and he won't let you die by other means, because otherwise it's not murder-by-spoon, but it does need to be murder-by-spoon.

If you watch all the extra "episodes," the real injury that occurs is caused by a spoon putting you in just the right place at just the wrong time. When being tormented by a Ginosaji, do NOT attempt suicide by bus.
