So of the old-school companies, who's left independent? Just SJ Games?

Flying Buffalo is still out there under rRick Loomis. Palladium is still running under Kevin Siembieda.

Does Amarillo Design Bureau qualify as a remaining old-school independent company?

I never really played Star Fleet Battles. I kind of want to try the simplified Federation Commander, but didn't pick it up when it was circulating in the FLGS several years ago.
