Default Determining surprise

The rules say it is the GM's job to determine surprise. How would one do this?

GM Fiat, or, as I prefer to call it, "Because I say so."

Basic Set, pg 393, gives guidelines on when to use the different kinds of surprise.

yes but how does one go about determining when they are "frozen"?

If the GM rules it is a Total Surprise and the character doesn't have Combat Reflexes? They are "frozen" (looping Do Nothing maneuvers) for 1d seconds.
