I like this list but I think the Bane is too powerful. It lets the ninja tailor an extremely effective attack to anything she encounters. I'd let a Ninja with Higher Purpose (Slay X) get Xbane for no extra charge, though.

Those are all fine, but it excludes some explicitly included items, including Shuriken. Ammunition for the fukumi-bari, bows, and blowguns makes sense, too, since a Ninja Tool expenditure is overkill to get them. I'd allow the items from DF8, why not.

I'd leave this up to the GM. I personally have no issue with it. Whipping out a Bane poison you just happened to have with you seems reasonable enough.

Right, and that does limit the gear a lot - you can't whip out your Ninja Tool and have it be some expensive item you don't carry but own, because of the Vow.

Ultimately, the limitation is "Anything the GM agrees is right on for a ninja," which is worth a measly -10% because ninja are awesome and can justify anything . . . even the Chuck Norris of pixies hidden in a special nageteppo!

Also, I think that the overlooked second paragraph is the kicker:
Alternatively, you can use your Gizmo to "trick out" a weapon of any size that you have ready in your hand: Your ninja-to's sheath might act as a snorkel or a blowpipe, your staff could have a hidden kusari inside or turn into climbing pole, etc. The limit here is one Gizmo per weapon per adventure. Thus, you must whip out a new weapon for each trick.
Yes, that does mean that if you have Gizmos 3 and go on four adventures, you end up with 12 permanent weapon enhancements. How is that not cool?

The problem with that is that says it includes Ninja Tools starting on page 15. Shuriken are on 14! Is this errata?

I agree, but I surprised at how short the list of stuff that you can use it to just get is, according to a literal reading!

There are no "errata" here! In GURPS, lists define rather than exemplify only when they explicitly say so. This list outright says "and so on," leaving it open-ended. Moreover, the text says, "any item under Ninja Tools (p. 15) or Covert Ops and Security Gear (see Dungeon Fantasy 1, p. 25) counts," and not, "only items under Ninja Tools (p. 15) or Covert Ops and Security Gear (see Dungeon Fantasy 1, p. 25) count." If we had intended the latter, we would have used that wording and not ended the previous list with an encompassing "and so on." What actually matters is:
  • The item is, in the GM's estimation, "ninja gear."
  • Weight per item cannot exceed Basic Lift/10.
  • Size is limited to Reach 1 or less and Bulk -4 or better.
  • If you haven't purchased and stashed the item, its cost cannot exceed $10 if permanent or $50 if expendable.
The first point is what the -10% limitation adds, and is intended to give the GM the right to forbid anything that's out of idiom, not to specify a short list. Another blanket rule in GURPS, similar to "examples are illustrative rather than definitive," is "the GM's word is final." This limitation gives a small cost break for the likely effects of that.

Can anybody explain to me why this advantage is leveled? I cannot seem to find any bonuses provided by higher levels.

Each additional level allows you to pull out another ninja tool that session.

Permanent permanent? As in if I trick out the saya on my ninja-to to also operate as a blow gun day one it will remain that way in perpetuity? Or it will remain that way until the end of the adventure? The former seems to be in the spirit of Gizmos in general, the latter seems somewhat open to having uber weapons/accessories after a while...
