In a DF game, had a player completely disarm himself with failed fast-draw rolls when trying to fight three giant spiders.

Character was a Scout/Knight build with heroic archer and weapon master.
First round, tries to ready his bow for a heroic archer shot, rolls 18 and drops bow.
Second round, tries to fast-draw his katana, rolls 18 and accidently flings it into the ceiling.
Third round, tries to fast-draw his short sword, rolls 18 and flings it into a wall.
Forth round, tries to fast-draw his knife, rolls 18 and flings it into the ceiling.
Fifth round, tries to fast-draw an arrow, rolls 18 and drops the whole quiver of arrows to the ground.
The party's Barbarian, Swashbuckler, Martial Artist, and Wizard weren't doing much better with normal misses. The party Cleric managed to save the day, after having All-Out-Defended for two rounds, each blocking two attacks, went offensive and did three critical hits in a row, each dropping a giant spider.

On other occasions, I've had monsters refuse to die, having not failed several HT rolls in a row while being driven down to within 2 or 3 HP of -5xHT HP.

Given the penalties on consciousness rolls it shouldn't have mattered.
