Even then, it's doable for a sufficiently well-protected fighter. On its own, -4 isn't a worry.
Just as commited attack, it should prohibit it as well.
Doubt that, with -4, even with combat reflexes you need an exceptional warrior(skill 18 per TS) to defend on 9-,with another weapon.

Can't do that, you're moving, and a moving target is harder to hit.

You could introduce a scale, like a reverse deceptive attack. For every -4 to your defences you take, down to a minimum of 3, you get one of the bonuses of all out attack. So -8 to defend would allow you to make 3 attacks (all out attack double twice).

What does that have to do with defending? If someone attacking is harder to hit, that's a penalty to hit. Not withstanding the fact that someone doing AOA might not move, or even be moving to attack - AOA (Determined) with a ranged weapon is clearly meant to represent a shooter making no attempt at all to move or react to attacks except by maximizing his hit chances.

I'm sorry, I just see a real, actual need for "does not defend at all" as a level of attack, and no amount of penalties will replace that need.

I'm not sure to what purpose, though. Where does setting a definition of "more than -10 means you can't defend" get you? Presumably you mean native penalties not accrued penalties, since otherwise Feint and Deceptive Attack can turn into automatic ways to bypass defenses, given enough skill. Or a defender in a bad spot (used Move and Attack and then got stunned for a total -6, so use DA -4 to hit and he gets no defense.)
