The Wizard's Lab
The adventurers finally get to the wizard's lab. When the room itself attacks them. A large magical TL 4/5 surgical laboratory. Restraints animate and seek to catch anyone close enough. Surgical tools fly into the air and attack. Half finished creations lunge from cages.
Symbol drawing and Thaumaturgy checks will reveal the haphazard and frequently modified workshop type enchantments that cover the walls and floor are connected to a single power source. Physical attacks on this will disrupt all the enchantments in the room.

This shapeshifting creature patrols the wilderness around the wizard's dungeon. It epitomizes the characteristics of different successfully predators in both its forms. It's first form is that of an enormously powerful stealthy solitary apex predator. It's second form is that of a large pack of smaller predators who use keen senses, cooperation and extreme endurance to hunt their prey.

Notes
- feel free to add a 3rd humanoid form
Hunter-Killer?
After a quick internet search to explain why that popped into my head...
In military jargon Hunter Killer Team is one that separates the tasks of finding targets and attacking them, for handling by separate components of the team. One example is the spotter sniper team.
This monster just acts as both members.

Over the next few seconds the body morphs smoothly to that of the demon. (The demon is fully active in this time)

Agent Smith.
Remember how any nearby innocent human could be used as source material for one of the Agent programs?

Sounds good, one tiny change though, add a "The" to it.
The Hunter-Killer.

Working off the reasoning that the more powerful/successful the predator the bigger it's range is. This one has a very big range.

Delorfan Goose

Delorfan was an ambitious mage with just a touch of practicality (but still no common sense) who got tired of feeding his magic guardians raw meat. It was expensive, and he didn't like the idea of undead or golems, so he set out to make a monster that could subsist on grain. The result: The Dolorfan Goose.

These geese are 7 feet tall, armored in steely black hard feathers, and have over grown talons on their webbed feet. And they breathe fire. And Dolorfan made good and sure they can still fly.

The result is a reasonable approximation to a poor man's dragon. With feathers, a long neck and brain of a very aggressive bird. And the fact that they eat grain just makes them more likely to stick around the peasant's fields instead of burning them and moving.

I'd make The Hunter-Killer as a construct built for assassination.

Tongued Stalagmite

Tongued Stalagmites are lumpy, bumpy creatures that hang out on ceiling caverns, looking like nothing so much as lumps of rock with Stalagmites hanging off. Don't be fooled! they are vicious predators that can shoot their stony spikes out like a chameleon's tongue. All of the spikes, which catch some folks off guard.

They see in the infrared range, and target accordingly, though it will take a truly learned delver to know that little fact. They have a hard rocky exterior, and though they have organs, it can be hard to tell exactly where those organs are.
