Matrix, you have given me the answer! On MA.109 that you refernce, the bullet point of attacks with limited reach. It spicificly says:
Attacks with limited reach. If a Combination involves an attack with fixed reach, that attack can only work if the preceding ones leave the target in range. In particular, if a closecombat strike like a punch or a Knee Strike follows an attack that hurls the victim one or more yards away – most often a shove, throw, or strike that inflicts knockback – the Combination ends if the attacker can’t step to deliver the short-ranged attack. And yes, 20 damage to a 10 HP target is likely crippling or lethal... unless that target has DR. If the Hulk is hammering on Iron Man with his fists, he'll want some way to keep him in place long enough to actually penetrate the suit.
