Default Fighting multiple Monsters: Combined, or one by one?

My first question is: When I kick open the door and I have to fight a monster and another player adds a monster, do I have to fight the monsters individually (i.e. one monster is a level 10, I beat it and then move on to the next monster which is a level 12 and then defeat it) or combined (monster #1 is a level 10 monster #2 is a level 12 and the combine and I have to beat their combine level of 22)????

For reference, this is covered on page 4 of the rules, under the heading "Fighting Multiple Monsters."
