Default [TG] One Foe option and Defenses

With the One Foe option introduced in Technical Grappling, you can benefit (and suffer consequences) from focusing on only one combatant.

If you fail the Per-4 roll to notice anything else, including attacks, around you, are you able to use active defenses against unnoticed attacks, or would these count as effectively surprise attacks?

No, of course you can't. You can only defend against attacks you are aware of. This is a standard rule.
