the bad stuff at Pterodactyl says that i have to loose my hand or all my small items. if i have no small items can i choose this option or i have to loose something? Also i must loose the small items from game or from game and small items from hand also?

If you do not have any Small items, you must discard your hand. And if you have no cards in hand, you must lose all your Small items. (If you don't have either, then you have bigger problems than one monster's Bad Stuff.)

When you're directed to lose cards, that ALWAYS means cards in play unless the text specifically talks about cards in your hand.
